New Patient Registration

You can register as a patient at Alexander House by completing a registration form and health questionnaire. You can also request for forms to be posted to you by calling 01942 482330 or collect in person at the surgery reception desk.

All newly registered patients will be asked to attend a registration health check. 

Alexander House welcomes all new patients, including asylum seekers, homeless people or members of the travelling community.

Medication is available from the date of registration.
